About PT-141

PT-141 (bremelanotide) is a melanocortin receptor agonist approved by the FDA under the brand name Vyleesi for hypoactive sexual desire disorder in premenopausal women. It works through the central nervous system rather than the vascular system, which makes it mechanically different from drugs like Viagra or Cialis.

It's used off-label by both men and women for sexual dysfunction and low libido. The typical dose is 1 to 2 mg injected subcutaneously about 45 minutes before desired effect. It's not a daily medication — you take it as needed.

Side effects include nausea (common with the first few doses) and temporary skin darkening due to its melanocortin activity. Most people find the nausea diminishes after a few uses.

Onset and what to expect

Effects typically begin 30 to 60 minutes after injection and can last 6 to 12 hours. This isn't like flipping a switch — it increases desire and arousal rather than producing a mechanical response. Some people respond strongly on the first dose, others need 2 to 3 uses before feeling the full effect.

The nausea is usually worst on the first injection. Many people take an anti-nausea medication the first time or start with a lower dose (500 mcg) to test tolerance.

Side effects

Nausea is the headline side effect, especially the first few times. It typically fades with subsequent uses. Flushing and headache are also common.

Skin darkening can occur with repeated use, particularly on the face. This is dose-dependent and reversible after stopping. The FDA labeling recommends no more than 8 doses per month.

Blood pressure elevation has been reported. If you have uncontrolled hypertension, this one's probably not for you.

Storage and reconstitution

Refrigerate after reconstitution at 36 to 46°F. Powder is stable at room temperature. Reconstitute with bacteriostatic water and use within 28 days.

Since PT-141 is used as-needed rather than daily, a single reconstituted vial lasts a while. Make sure you're still within the 28-day window.
